Pneumonia — Expected Recovery vs Treatment Failure

Clinical improvement often precedes radiographic clearing.

Know what should improve first

Fever, dyspnea, oxygenation and systemic symptoms should trend in the right direction before the chest x-ray fully normalizes.

Do not chase residual infiltrates too early

Radiographic abnormalities may persist for weeks, especially in older adults or more severe disease.

Treatment failure has a differential

Wrong pathogen, resistance, abscess/empyema, obstruction, aspiration, alternate diagnosis or host factors can explain poor response.

Reassess the patient, not just the film

Persistent fever, worsening oxygenation, hemodynamic decline or new complications matter more than an unchanged early x-ray.
