Recognizing decompensation is more useful than chasing a single BNP, weight, or edema finding.

Compare with baseline

Know the patient's usual weight, symptoms, oxygen need, edema, exercise tolerance, diuretic regimen and renal function.

Congestion is a clinical synthesis

Dyspnea, orthopnea, JVP, pulmonary findings, edema, weight trend, imaging and natriuretic peptides support the picture together.

Compensated vs decompensated matters

Stable chronic HF can have persistent abnormalities. Decompensation means the clinical state changed — not simply that the patient has HF.

Watch treatment tolerance

Renal function, BP, potassium and symptoms determine whether diuresis and chronic HF therapies remain safe during recovery.
