Not every epigastric symptom is 'just reflux.'

Pattern the symptoms

Postprandial burning/regurgitation suggests reflux; focal epigastric pain, NSAID exposure, or H. pylori risk may shift attention toward ulcer disease.

Screen for alarm features

GI bleeding, progressive dysphagia, persistent vomiting, unintentional weight loss, anemia, or concerning new symptoms change the evaluation.

Medication history matters

NSAIDs, aspirin, bisphosphonates, potassium, iron, and other agents can worsen upper-GI symptoms or mucosal injury.

Treat the mechanism

Acid suppression, H. pylori testing/treatment when indicated, NSAID risk reduction, and lifestyle measures should match the likely syndrome.
